JOB CENTER for migrant workers

Carexcell is recruiting a Care Manager to work in our growing Coffs Harbour office. This is an exciting opportunity to work in a fast paced, growing and passionate team.

The Role

Reporting to the Home Care Package Program Manager you will be responsible for supporting older people living with age related functional decline. On a day to day basis you will:

Meet with clients and prospective clients,
Undertake client assessments, develop care plans, budgets and agreements,
Engage with client's broader health team to ensure that their needs are being met,
Support client's to understand their Home Care Package, particularly package inclusions and exclusions.
Provide clinical care/services and advice to clients and staff,
Maintain documentation,
Support the recruitment, retention and management of frontline staff,
Have input into the organisation's continuous improvement and strategic objectives.
About You

You are a passionate self starter that is motivated by improving client outcomes. You have a great sense of humour and are able to engage with and influence people from all walks of life. You:

Are an AHPRA registered professional (RNs, ENs, Physios and Occupational Therapists are encouraged to apply!),
Have a current drivers license (car packaging available and corporate vehicle available for work purposes),
Have experience working with older people and people living with disabilities.
